Flight 1284 Pet Death:
This was a tragic accident and we take full responsibility.
We’ve spoken to the family, our crew and a number of passengers who were seated nearby, and we learned that the customer did tell the flight attendant that there was a dog in the carrier. However, our flight attendant did not hear or understand her, and did not knowingly place the dog in the overhead bin.
We remain in contact with the family to express our condolences and offer support.
To prevent this from happening again, by April we will issue bright colored bag tags to customers traveling with in-cabin pets. This visual tag will further help our flight attendants identify pets in-cabin.
Federal legislation introduced to ban animals from overhead bins:
Our existing policy is for in-cabin pets to be placed under the seat in front of the customer.

Dog switch (dog sent to Tokyo instead of Wichita):
An error occurred during connections in Denver for two pets sent to the wrong destinations.
The pets are on their way to be reunited with their owners.
We apologize for this mistake and are following up with the vendor kennel where they were kept overnight to understand what happened.

Our PetSafe team is trained and dedicated to do everything possible to ensure animals have a safe and comfortable journey while in our care. To meet this standard, we are meticulous about providing pre-flight instructions to pet shippers, requiring health certificates and ensuring all kennels and crates conform to IATA and USDA regulations.
United flew nearly 140,000 pets via our PetSafe program in 2017, far more than any other carrier. Independent medical experts determined that the overwhelming majority of the incidents were not attributable to United.
Additionally, in 2017 we formed a PetSafe Advisory Board, comprised of recognized veterinarians, experienced animal shippers, animal behaviorists and other experts, to make recommendations for steps we and our customers can take to enhance animal safety going forward.
