Incident: Mistral AT72 at Lampedusa on Aug 9th 2015, engine shut down in flight

A Mistral Air Avions de Transport Regional ATR-72-212A on behalf of Alitalia, registration I-ADLW performing flight AZ-1813 from Lampedusa to Palermo (Italy), was in the initial climb out of Lampedusa when the right hand engine (PW124) failed prompting the crew to join a downwind to return to Lampedusa. On downwind the crew received an engine fire indication, shut the engine down and continued for a safe landing back in Lampedusa.

The flight was cancelled.

Italy's ANSV reported the engine failed while climbing through 800 feet AGL. The occurrence was rated a serious incident, an investigation has been opened.

The occurrence aircraft is still on the ground in Lampedusa about 42 hours after landing back.

Inconveniente grave a Lampedusa




Il 7 agosto, intorno alle 21.15, l’aeromobile ATR 72 marche I-ADLW, in decollo da Lampedusa con destinazione Palermo, subiva, a 800 piedi, un’avaria al motore n. 2, che comportava il rientro in emergenza sull’aeroporto di partenza. L’atterraggio avveniva regolarmente.
Acquisite tutte le prime evidenze, l’evento è stato classificato come inconveniente grave, con conseguente apertura, da parte dell’ANSV, dell’inchiesta di sicurezza.
 
Lo stesso che qualche settimana fa ha avuto fumo in cabina ed è atterrato a Trapani.

A Mistral Air Avions de Transport Regional ATR-72-212A on behalf of Alitalia, registration I-ADLK performing flight AZ-1808 from Palermo to Pantelleria (Italy) with 16 passengers and 4 crew, was enroute near Trapani (Italy) when smoke was seen in the cabin prompting the crew to divert to Trapani for a safe landing.

The cause of the smoke in the cabin is under investigation.
