A Lufthansa Airbus A340-600, registration D-AIHH performing flight LH-730 (dep Jan 10th) from Munich (Germany) to Hong Kong (China), encountered fumes on board enroute. On approach to Hong Kong the flight crew donned their oxygen masks and continued for a safe landing in Hong Kong. At least one of the pilots reported sick feeling unwell as result of the fumes and went to a hospital.
Maintenance subsequently checked the aircraft and identified the APU as source of the fumes, replaced the related filters and released the aircraft to service for the return flight.
During the return flight LH-731 the aircraft encountered another fumes event, see Accident: Lufthansa A346 enroute on Jan 11th 2016, fumes on board causing a flight attendant to be given a sick note by doctors.
The aircraft returned to service following the return flight and encountered a third fumes event on the third consecutive flight.
First sources reporting these two events on LH-730 and LH-731 hinted to The Aviation Herald, that the internal documentation by Lufthansa was reporting the occurrences, sources reporting later however stated, that there were only entries for the aircraft on the date of Jan 11th 2016 stating "no data".
On Jan 18th 2016 Germany's BFU told The Aviation Herald that all three events have been reported to the BFU, currently data are being collected to establish the severity of the events for classification of the occurrence either as incident, serious incident or accident.

A Lufthansa Airbus A340-600, registration D-AIHH performing flight LH-731 (scheduled dep Jan 10th, actual dep Jan 11th) from Hong Kong (China) to Munich (Germany), was enroute when fumes were noticed on board, followed by a number of flight attendants reporting feeling unwell with dizziness, head ache, eye and throat irritation. The flight continued to Munich for a safe landing on schedule. After landing the affected crew members went to undergo medical examinations, one passenger also reported sick as result of the fumes during the flight and went to see a doctor.
The occurrence aircraft remained on the ground for about 16 hours, then resumed service.
The Aviation Herald received information that following the return from Hong Kong the aircraft received new carpets and other textile equipment, the crew of the next flight was thus advised to expect associated smells including plastic smell during the first stages of flight.
The occurrence was the second of three consecutive flights encountering fumes and causing injuries to crew, see the prior flight to Hong Kong at Accident: Lufthansa A346 enroute on Jan 11th 2016, fumes on board and the subsequent flights to Dubai at Accident: Lufthansa A346 enroute on Jan 12th 2016, fumes on board.

A Lufthansa Airbus A340-600, registration D-AIHH performing flight LH-638 (dep Jan 11th) from Munich (Germany) to Dubai (United Arab Emirates), encountered fumes during the flight prompting the crew to don their oxygen masks on approach to Dubai. The aircraft landed safely in Dubai, at least one of the pilots however needed medical care after the return to Munich and went on sick leave as result of the fumes event.
The aircraft had encountered fumes during the previous two sectors, rotation to Hong Kong, too, see Accident: Lufthansa A346 enroute on Jan 11th 2016, fumes on board and Accident: Lufthansa A346 enroute on Jan 11th 2016, fumes on board.
The aircraft had remained on the ground in Munich for 16 hours following the landing of the previous sector from Hong Kong. According to sources the crew of flight LH-638 was advised to expect unusual odour including plastic smell due to renewal of textile environment (carpets etc.).
Sources suggest that there is no write up regarding flight LH-638 in Lufthansa's internal technical documentation.
The occurrence aircraft returned to Munich as flight LH-639 on Jan 12th 2016 on schedule and was removed from service afterwards. The Aviation Herald was informed that the APU was replaced. The aircraft returned to service in the evening of Jan 14th 2016 after about 54 hours on the ground in Munich.
On Jan 18th 2016 Germany's BFU confirmed to have received information on that occurrence, data are being collected to permit proper classification of the occurrence.
