Incident: TAM A332 at Paris on Oct 10th 2012, engine shut down in flight

A TAM Linhas Aereas Airbus A330-200, registration PT-MVE performing flight JJ-8055 from Paris Charles de Gaulle (France) to Rio de Janeiro Galeaom,RJ (Brazil) with 201 passengers and 16 crew, was in the initial climb out of Charles de Gaulle's runway 09R when the right hand engine (PW4168) emitted a loud bang, the engine's fire indication activated and the cabin lights briefly extinguished. The crew stopped the climb at 3000 feet, shut the right hand engine down, discharged one bottle of fire extinguisher and returned to Charles de Gaulle's runway 09R for a safe overweight landing about 15 minutes after departure.

Passengers reported they heard a loud noise like an explosion immediately after takeoff, the cabin lights briefly extinguished, and the aircraft stopped climbing. The captain subsequently announced the right hand engine had failed and was shut down, they were returning to Paris.

The airline confirmed the aircraft returned to Paris due to an engine problem. The passengers were accomodated in hotels while waiting for a replacement aircraft.

A replacement Airbus A330-200 registration PT-MVB departed Paris as flight JJ-9355 the following day and reached Rio de Janeiro with a delay of 20.5 hours.

The engine's low pressure rotor was found seized, a visual inspection of showed the low pressure turbine blades at least of the last turbine stage damaged with metallic debris and oil in the engine exhaust.
