Incident: ItAli Airlines MD82 at Milano on Feb 10th 2010, engine failure

By Simon Hradecky, created Friday, Feb 12th 2010 11:35Z, last updated Saturday, Feb 13th 2010 09:53Z

An ItAli Airlines McDonnell Douglas MD-82, registration I-DAWZ performing positioning flight FS-9609 from Milano Malpensa to Naples (Italy) with 6 crew, was just before rotation on Malpensa's runway 35R in night conditions, when the right hand engine (JT8D) began to emit sparks. The aircraft continued its takeoff, climbed to a safe altitude and returned to Malpensa. ATC offered runway 35L for the return and initiated a runway inspection of 35R, bringing the airport to a complete stop for about 15 minutes. The aircraft landed back onto runway 35R about 15 minutes after takeoff.

Following the landing runway 35R remained closed until debris was removed from the runway.

A post flight inspection revealed, that the engine inlet had collapsed possibly as a result of foreign object damage (ice) and parts of the inlet had gone through the engine core causing extensive damage to the engine and associated hydraulics system.

Italy's Agenzia Nazionale per la Sicurezza del Volo ANSV opened an investigation into the occurrence stating, that the right hand engine exhibited severe vibrations. Extensive damage occured inside and outside the thrust reverser engine #2.
